1. Initially when we have scheduled an appointment date and time, I will ask you for all insurance information in order to pass onto my medical biller who will verify your coverage. In the event that I haven't obtained that information from you please contact my biller directly at 1-775-267-6972 and give her all of your insurance information; you can also click on the Helpful Forms tab, scroll down to the Insurance Verification Form fill it out and email to me ASAP. Either way your coverage will be verified including all benefits, number of visits, and co-pay information. Prior to confirmation of your appointment, I will send you a reminder of the session and any pertinent info with regard to that coverage .
If you have EAP benefits thru your employer, you will need to contact them directly in order to obtain an authorization number including the number of visits, etc.
Please send me any authorization info, etc once you have obtained it prior to our initial session.
2. Please click the Helpful Forms tab and begin to fill out all forms listed on that page. You can email them to me at least 48 hrs prior to your initial session,or print all forms, fill them out, and bring them to your initial session. I will contact you 48 hrs prior to your initial session in order to confirm your appointment.
The bottom 2 forms have special instructions, and if they do not apply to you please disregard them.
3. Please click the Contact Me tab where you will find a map for my location. I am near The Spectrum Mall in Irvine, CA. 92618
If for any reason you are unable to keep your appointment, please contact my office at : 949-453-9600 at least 24 hours before your scheduled appointment.
